We look forward to welcoming you to Sunnybank Eye Doctors. To help your visit run smoothly, please take a few moments to read through the important information below.

Our Location

Suite 27, McCullough Centre
259 McCullough Street, Sunnybank QLD 4109
(Co-located with Sunnybank Private Hospital)

There is free on-site parking available. Please allow extra time to find parking, as the car park can be busy at times. A patient drop-off zone with ramp access is available at the building entrance, and lift access is available from the carpark.

Appointment Duration

Please allow up to 2 hours for your initial consultation. This allows time for a comprehensive assessment, any required diagnostic tests, and dilation of your pupils (if necessary).

New Patients & Registration

If you're new to our clinic — or haven't visited in some time — please complete our Patient Registration Form before your appointment if you can.  We will send you a copy prior to your appointment.

We kindly ask that you:

  • Complete and sign the form prior to your appointment

  • Email the form to our reception team in advance (details will be provided), or

  • Bring the completed form with you on the day

If you're unable to complete the form ahead of time, please arrive early to allow time to fill it in at the clinic.

Referrals

valid referral is required to claim a Medicare rebate.

Please ensure we have a current referral before your appointment. This can be sent:

  • Directly from your GP or optometrist, or

  • Provided by you via email or in person

Referral validity periods:

  • From a GP or optometrist: 12 months (from the date of your first specialist appointment)

  • From another specialist: 3 months

Please note: If you are attending for a new condition, Medicare requires a new referral, even if you have a valid one for another issue.

We’ll do our best to remind you when your referral is about to expire, but it is your responsibility to ensure a valid referral is in place to claim Medicare rebates.

What to Bring

To help us provide the best care, please bring:

  • A list of all current medications, including any eye drops or ointments

  • Your current prescription glasses or spectacles

  • Your Medicare cardPension or DVA card, and Private Health Insurance card (if applicable)

Dilating Eye Drops & Driving

Your doctor may use dilating drops to examine the back of your eyes. These drops can cause temporary blurred vision and light sensitivity for several hours.

We strongly recommend you:

  • Do not drive after your appointment if dilating drops are used

  • Arrange for someone to drive you, or

  • Plan for safe alternate transport (e.g. taxi, rideshare, friend/family)

Consultation Fees

Fees vary depending on the services you require and we will provide you with more information at the time you book your appointment.  Additional fees may apply for further diagnostic tests or procedures performed on the day. These will be discussed with you at the time.

Payment Information

All consultations must be paid on the day.
We accept:

  • EFTPOS

  • Visa/MasterCard

We do not accept cheques, American Express or Diner's Card.

Appointment Reminders

We will send you a text message reminder a few days before your appointment.
If you need to reschedule or cancel, please notify us as early as possible so we can offer the appointment to another patient.

Interpreter Services

If you require an interpreter, please let us know at the time of booking so we can arrange this for you in advance.

If you have any questions or need further assistance before your appointment, don’t hesitate to contact our friendly team. We’re here to help and look forward to seeing you soon.
